The Astra Dent network believes that not all wisdom teeth require mandatory removal. But there are clinical situations when postponing can lead to complications. Here are the indications for which surgeons advise not to delay the intervention:

  • Pain in the ear or jaw. The pressure of the impacted tooth on the nerve endings can cause pain to radiate to the temporomandibular joint or even the ear.
  • Pericoronitis. This is inflammation of the gums around a partially erupted wisdom tooth. It is accompanied by swelling, pain, an unpleasant odor and an increase in temperature.
  • Pressure or destruction of neighboring teeth. An incorrectly positioned wisdom tooth often presses on the seventh molar. This causes caries or leads to loosening of neighboring teeth.
  • Lack of space or trauma to the mucous membrane. If the jaw is too small, the tooth grows at an angle, injures the cheek or tongue, which causes painful sores and constant discomfort.
  • Retinal or dystoped teeth. When the wisdom tooth remains completely in the bone or erupts in the wrong direction, it does not participate in chewing, but causes chronic inflammation and bite disorders.

Levels of complexity of wisdom tooth extraction

In the Astra Dent network, the final cost of the procedure depends not only on the materials or type of anesthesia, but also on the level of complexity of the extraction. Doctors preliminarily assess the situation based on a clinical examination and 3D-CT to determine which of three levels the case belongs to:

  • Simple extraction. The tooth is completely erupted, does not have curved or branched roots, and is well fixed in the jaw. The procedure lasts 10-15 minutes, and almost does not require postoperative treatment.
  • Semi-retained extraction. Part of the tooth remains in the bone or is covered by the gums. The surgeon needs to make an incision in the tissues, possibly sawing the crown or root. Requires precise planning and takes more time.
  • Fully impacted or atypical tooth. Such a tooth has not erupted, is located horizontally or at an angle, often in a hard-to-reach area. Removal is carried out by dissecting the tissues, removing part of the bone, and sometimes suturing. This is the most complex category, requiring experience, precise equipment and postoperative support.

Wisdom tooth removal process

First, the dentist-surgeon examines the patient and prescribes a 3D-CT or panoramic X-ray. This is neces sary to determine the position of the roots, the degree of eruption, and the proximity to the nerve. Such diagnostics allow us to avoid complications during the operation and accurately plan the course of the intervention.

Choice of anesthesia

The Astra Dent network uses several options for anesthesia:

  • Local anesthesia is standard for most cases, acts quickly and effectively.
  • Computer anesthesia STA is ideal for patients with increased sensitivity or fear.
  • Drug sedation is recommended in complex or long-term cases, as well as for patients with anxiety. The person is in a relaxed state, but conscious.

Surgical intervention

Removal can be:

  • Simple, when the tooth is completely erupted and has a normal anatomical location.
  • Difficult, if the tooth is impacted, partially or completely covered by the gum or bone, or has curved roots. In this case, a gum incision is performed, sometimes – root sawing or crown fragmentation for phased removal.

Suturing

After a complicated removal, the surgeon sutures the wound using self-absorbable or removable sutures. This promotes faster healing and reduces the risk of infection.

After the operation, the doctor gives clear recommendations:

  • You should not eat for the first 2-3 hours. Do not touch the area with your tongue or fingers.
  • Do not rinse your mouth for 24 hours, avoid hot food and drinks.
  • Avoid physical exertion, bending, flying for 2 days.
  • If there is swelling, apply cold.
  • If there is pain, take recommended medications. If necessary, the doctor prescribes antibiotics.
  • A follow-up examination is after 5-7 days. If there were sutures, they are removed or the healing is checked.

  • Cold. In the first hours, apply ice to the cheek for 10-15 minutes with breaks. This reduces swelling and pain.
  • Diet. Avoid hot, hard, spicy food. In the first days, soft food at room temperature is recommended – pureed soups, yogurts, porridges.
  • Hygiene. Brush your teeth carefully, avoiding the area of the intervention. Do not rinse actively for the first day. Then use antiseptic solutions as prescribed by the doctor.
  • Medications. Take painkillers or antibiotics if prescribed by the surgeon. Do not change the dosage without consultation.

Contact the department if severe pain persists for 3-5 days, pus appears, the temperature rises sharply or there is severe swelling. These may be signs of inflammation or dry socket that require professional intervention.
