Below we will consider exactly how root canal treatment is performed in young patients step by step.

Diagnosis: the first and key stage

Before the start of endodontic intervention, the pediatric dentist conducts:

  • Clinical examination – the doctor assesses the general condition of the tooth, the presence of caries, swelling of the gums, the child’s complaints of pain or sensitivity.
  • X-ray diagnostics – a targeted image is taken, which allows the doctor to see the structure of the root, the depth of the lesion, the presence of changes in the periapical zone.
  • Cone-beam computed tomography (CT) – is used in complex or controversial cases, in particular when multiple canals or infectious complications are suspected.

Choosing a method: amputation, extirpation or mummification?

Three main methods are used in pediatric endodontics:

  • Amputation (vital/devital). This is the removal of only the coronal part of the pulp (nerve), when the root canals are not yet affected. The method allows us to preserve the viability of the root pulp. It is performed in cases of superficial lesions without periodontal changes.
  • Extirpation. Complete removal of the pulp from the crown and roots of the tooth. Indicated for deep inflammation or necrosis of the pulp. Performed under a microscope or with magnification to ensure accurate cleaning and eliminate relapses.
  • Mummification. A technique that involves applying a special preparation to the affected pulp with its subsequent “preservation”. It is used only in exceptional cases when other options are unavailable or have contraindications. At Astra Dent, this method is practically not used, since its effectiveness is inferior to modern approaches.

Pain relief: complete safety for the child

All stages of treatment are painless, taking into account the individual reaction of the child and the type of intervention. Astra Dent offers:

  • Application anesthesia – superficial anesthesia in the form of a gel or spray, used before the injection or during enamel treatment.
  • Infiltration and conduction anesthesia – injected into the gum near the diseased tooth, providing insensitivity. It is selected depending on the localization of the tooth.
  • Treatment under medical sleep – recommended for children with a high level of anxiety, as well as for long or extensive interventions. Inhaled or intravenous drugs are used under the control of an anesthesiologist.

Modern means provide the onset of action after 30-60 seconds, the child does not feel pain, and after the procedure quickly returns to their usual state.

Control systems during sedation

During treatment, the condition of the small patient is carefully monitored by anesthesiologists using high-precision equipment. In real time, the following are monitored:

  • blood oxygen saturation level (pulse oximetry),
  • heart rate via ECG,
  • respiration and blood pressure indicators,
  • depth of drug-induced sleep thanks to BIS-monitoring – a special technology that analyzes the electrical activity of the brain.

This comprehensive control allows the doctor to immediately respond to the slightest changes and ensure the maximum level of safety for the child.

Canal treatment: sterility and accuracy

After anesthesia, the doctor performs the following stages of canal cleaning:

  • Mechanical treatment – the canal is expanded using special flexible instruments that take into account the structure of milk teeth. Astra Dent uses nickel-titanium files with depth control.
  • Antiseptic treatment – rinsing with a hypochlorite or chlorhexidine solution is performed, which destroys bacteria inside the canal.
  • Interoperative control – in some cases, an apex locator or control image is used to ensure complete cleaning to the physiological apical opening.

All equipment is sterilized according to protocols approved by the Ministry of Health and international standards.

Canal filling: protection against infection

After cleaning the canals, they are hermetically sealed:

  • Biocompatible materials are used – pastes based on calcium hydroxide or iodoform, which have bactericidal properties.
  • The materials are easily removed when changing the tooth – the filling does not interfere with the growth of the permanent tooth and does not cause allergies.
  • Filling is performed under a microscope or with optical magnification – this allows you to fill even the thinnest canals without voids.

The final stage is the restoration of the tooth crown with a filling material or onlay.

Post-procedural care: recommendations for parents

After treatment, the doctor must explain to the parents how to care for the tooth:

  • Refrain from hard or hot food for 1-2 hours after the procedure.
  • Maintain oral hygiene, but avoid intensive brushing near the treated tooth in the first few days.
  • Attend follow-up examinations according to the schedule set by the doctor (usually every 3-6 months).

At the slightest changes in the child’s behavior, for example, in case of repeated pain or swelling, immediately consult a specialist.
